Effective primary care networks have good governance

Primary care networks (PCNs) are now firmly established as an important part of the delivery of primary care across England. There continues to be significant central investment in PCNs which, of course, comes with an increasing number of objectives ranging from improving prevention and tackling health inequalities to delivering increased patient access, one part of the latter bringing together existing PCN extended hours funding with extended access services commissioned by the former clinical commissioning groups. All of these asks require PCNs to not only establish, but continually review their governance arrangements.

Running an effective primary care organisation is challenging even at times of relative stability, but we are seeing today that the status quo does not exist in primary care and one of the main challenges facing PCN boards is to ensure that their governance systems are in a position to deal with disruption and a continually changing ask. Good governance will add value to any organisation in a number of areas. It will support the organisation as it deals with the operational challenges that present as a result of change and do so in a way that reflects its vision and values. As good governance has the interests of all stakeholders at its heart, both internal and external relationships benefit from the confidence that it brings. This of course, is especially important now as we develop integrated care systems and encourage closer collaborative relationships with others across the system.

It is important therefore that every PCN board sets aside time regularly to review the key elements of its governance arrangements which will require an honest appraisal, collectively and individually, of how well it functions on key areas. The type of things this appraisal needs to cover include data sharing, decision making, service delivery, infrastructure – boards, roles etc., operational policies, finance, the network agreement and staffing.

This approach supports and encourages an open and honest organisational culture, and by recognising the important role good governance plays on safety, quality and delivery, sets solid foundations for the success of any PCN.
